Why Partnership Is Key

Meet Our Outstanding Partners

Neighborhood Networks creates new connections between healthcare organizations and our network of community-based organizations for the purpose of meeting the health and social needs of community members. Our network of community-based organizations, and the community health workers they employ, are the engine that drives our success.

HUB

Healthcare Organizations

Healthcare organizations refer their members who have health and social needs to receive care coordination services designed to help improve health outcomes.

HUB

Neighborhood Networks links referred clients to community-based organizations and provides administrative and management services: contracting, training, quality assurance, reporting, and secure online care management systems.

Community-based Organizations

Community-based organizations hire and supervise community health workers, who assess clients’ health and social needs, provide support and education, and connect clients with needed services.
