Our network of community-based organizations, and the community health workers they employ, are the engine that drives our success.
Healthcare Organizations
Healthcare organizations refer their members who have health and social needs to receive care coordination services designed to help improve health outcomes.
HUB
Neighborhood Networks links referred clients to community-based organizations and provides administrative and management services: contracting, training, quality assurance, reporting, and secure online care management systems.
Community-based Organizations
Community-based organizations hire and supervise community health workers, who assess clients’ health and social needs, provide support and education, and connect clients with needed services.
Our Network Partners
Chula Vista Community Collaborative (CVCC) works with partner organizations to share information and resources that strengthen families and communities. CVCC elevates the needs and voice of the community and works to ensure seamless access to needed services.
Social Advocates for Youth (SAY) San Diego partners with youth, adults, and families to help them reach their full potentials. With a vision of opportunity, equity and well-being for all San Diegans, SAY San Diego’s holistic approach enriches youth, empowers individuals and families, and engages communities.
North County Lifeline serves low-income and underserved populations with clinically strong, evidence-based programs. North County Lifeline builds self-reliance among youth, adults, and families through high-quality, community-based services.
