Neighborhood Networks is housed at the San Diego Wellness Collaborative, a nonprofit organization that brings together stakeholders from multiple sectors to improve community health and advance health equity. The San Diego Wellness Collaborative serves as the backbone organization for three primary initiatives:

• Be There San Diego is a coalition of patients, communities, and healthcare systems working together to prevent heart attacks and strokes.

• San Diego Accountable Community for Health (SDACH) is a groundbreaking initiative for collaboration across sectors to create community wellness and improve health equity.

• Neighborhood Networks was conceived in part by the SDACH and serves as a Medi-Cal Community Care Hub to connect managed care and community-based organizations to address the health-related social needs of community members.
