Our network of community-based organizations, and the workforce they employ, are the engine that drives our success.
Healthcare Organizations
Managed care plans refer their Medi-Cal members who have health and social needs to receive care coordination services designed to help improve health outcomes.
HUB
Neighborhood Networks connects referred clients to care coordinators and provides administrative and management services: contracting, claims and billing, training, workforce supports, quality assurance, reporting, and secure care management system.
Community-Based Organizations
Community-based organizations hire and supervise care coordinators, who assess clients’ health and social needs, provide support and education, and connect clients with needed services.

Make a Referral
Refer individuals to Neighborhood Networks by submitting our referral form. Simply provide us with basic information and our team will take it from there to confirm eligibility and make a warm hand-off to a care coordinator. We’ll also follow up to let you know how things are going.
