Challenge:
Roger was going through a difficult time in his life. Dealing with a recent divorce, he found himself in a vulnerable situation living in an RV with his dog. Roger’s health had declined significantly. He struggled with chronic pain, mobility problems, and insomnia and was at risk for institutionalization.
Outcome:
Neighborhood Networks is helping people just like Roger. Neighborhood Networks deploys a community-based workforce—including care managers—to support people who have unmet health and social needs and connect them with available resources. Workforce members are employed by trusted, community-based organizations and meet clients where they are, whether at home or in their neighborhoods.
Roger was referred to Neighborhood Networks for support and was connected with a care manager who provided the following assistance:
- Reconnected Roger with his healthcare providers to assure he received treatment
- Helped him address his insomnia
- Connected him with housing assistance to find an affordable place to live
- Helped him apply for a program that provides specialized care and ongoing support from a R.N.
After months of working together with his care manager to establish health goals, Roger was able to successfully move into his own one-bedroom apartment and continues to receive ongoing support.
