What is Neighborhood Networks?

Neighborhood Networks is a Medi-Cal Community Care Hub that connects Medi-Cal managed care plans with community-based organizations (CBOs) to address members’ health-related social needs through no-cost Medi-Cal services. Neighborhood Networks manages activities such as contracting, authorizations, claims, a shared case-management system, training, and quality assurance, while CBO partners employ the community-connected workforce and deliver services.

What is Enhanced Care Management (ECM)?

ECM is a Medi-Cal benefit that gives eligible members an added layer of support in addition to their regular care. Once enrolled, Medi-Cal members receive assistance from a Lead Care Manager, who coordinates medical, behavioral, and social services; helps with transitions of care; promotes self-management; and connects members to community resources.

What are the eligibility requirements for ECM?

Members must be enrolled in Medi-Cal and meet criteria for specific ECM services. Some examples include being at risk for homelessness, having a serious physical or mental health condition, and having multiple recent visits to the hospital or emergency department. There are other ways to be eligible, depending on the unique ECM service.

What is Community Supports (CS) Housing?

CS Housing is a Med-Cal benefit that connects eligible members with a Housing Navigator, who helps individuals find housing, get needed documents, and sign up for available benefits. The Housing Navigator may also help the member apply for financial assistance to help with initial move-in costs such as security deposits, utilities, and set-up fees, and can also support individuals to maintain housing up to 12 months.

What are the eligibility requirements for CS Housing?

Members must be enrolled in Medi-Cal, be experiencing homelessness or at risk for homelessness, and meet other health-related needs.

Which Medi-Cal managed care plans does Neighborhood Networks work with?

Neighborhood Networks works with four Medi-Cal managed care plans in San Diego County—Blue Shield Promise, Community Health Group, Kaiser Permanente, and Molina Healthcare—and two managed care plans in Imperial County—Community Health Plan of Imperial Valley/Health Net, and Kaiser Permanente.

Can organizations refer an individual to Neighborhood Networks?

Yes. Organizations can refer to Neighborhood Networks by submitting a referral form here or using the QR code on the Neighborhood Networks website. They can also call 619-273-3295 to speak directly with an outreach and referral team member.

Can individuals refer themselves directly into Neighborhood Networks?

Yes. Individuals can self-refer to Neighborhood Networks by submitting an interest form here or using the QR code on the Neighborhood Networks website. They can also call 619-273-3295 to speak directly with an outreach and referral team member.

What happens after a referral is made to Neighborhood Networks?

When a referral is received, the Neighborhood Networks outreach and referral team verifies Medi-Cal eligibility and obtains authorization, then makes a warm hand-off to a CBO-employed Lead Care Manager and/or Housing Navigator, who contacts the individual to begin services.

Will any Medi-Cal benefits change if someone is enrolled in ECM or CS Housing?

No. ECM and CS Housing services are part of members’ Medi-Cal benefits and do not change their current doctors or coverage.

Can a person receive ECM and CS Housing services at the same time?

Yes. Eligible Medi-Cal members may participate in ECM and CS services at the same time. For example, CS Housing Navigators can support members with housing-specific needs, while ECM Lead Care Managers can support the same members with other services such as physical and behavioral health care coordination and connection to social services. If a member receives both services within the Neighborhood Networks partner network, the Housing Navigator and Lead Care Manager can work together to coordinate care.

Where are Neighborhood Networks ECM and CS Housing services provided?

Experienced ECM Lead Care Managers and CS Housing Navigators are employed by our network of trusted, local community-based organizations. Workforce members are from the communities they serve, provide services in multiple languages, and have exclusive knowledge of local resources. ECM and CS Housing services may be provided over the phone or in person, at home or in the community.
